This note is to give you a brief update. Our office received an e-mail on 11-17-04 confirming the advance request and forwarding to the appropriate department. I had to travel to Tennessee due to a death in the family on 11-18-04.. I have briefed my claims manager on your situations. I also checked the information from your website but I will probably need more details on contents..
